About HealthSense Medical & PharmaSense
In 2006, a group of pharmacists, doctors, and nurses collaborated to bring a patient focused practice to life. Our aim was to eliminate silos and provide excellent primary care. In addition to Family Practice and a Walk-In Clinic, we added Travel Medicine, which was under-served at the time. We became a Certified Yellow Fever Vaccination Site by Health Canada and helped keep our community safe from disease. Later we hired Alim Janmohamed, an amazing pharmacist and manager, who brought his Diabetes specialty into the practice and oversees the Medical Clinic.
Fifteen years later, we continue to bring attentive healthcare to Oakville residents.

Alim Janmohamed, R.Ph., CDE, Clinic Director
Alim Janmohamed is the Clinic Director at HealthSense Medical and designated pharmacy manager at PharmaSense Pharmacy. Alim obtained his bachelors degree from McMaster University in 2006, and graduated from the Leslie Dan Faculty of Pharmacy at the University of Toronto in 2010. Upon graduation, Alim worked at St. Joseph’s Hospital in Hamilton, and then at Costco Pharmacy where he provided a variety of clinical services. Alim has worked at PharmaSense since 2012.
Alim is also a Certified Diabetes Educator, and provides consultative services to diabetic patients at PharmaSense and HealthSense. He oversees the Ideal Protein Weight Loss program at PharmaSense, is certified to administer injections and immunizations, and is also a certified fitter for compression socks / stockings. Alim is dedicated to providing quality healthcare in a collaborative setting.

Sarah Stanlake, R.P.N., Clinic Manager
Sarah is a Registered Practical Nurse in good standing by the College of Nurses of
Ontario. She provides excellent care and serves as Clinic Manager of Health Sense Medical.

Dr. Shahbaz Khan, MD, CCFP
Licensed M.D. registered with The College of Physicians and Surgeons of Ontario, in good standing. Dr. Khan specializes in Family Practice and is certified by the College of Family Physicians of Canada. He is fluent in English, Hindi, Punjabi and Urdu

Dr. Samapti Samapti, MD, CCFP
Licensed M.D. registered with The College of Physicians and Surgeons of Ontario, in good standing. Dr. Samapti specializes in Family Practice and is certified by the College of Family Physicians of Canada.
Dr Sabahat Jafri, MD, CCFP
Licensed M.D. registered with The College of Physicians and Surgeons of Ontario, in good standing. Dr. Jafri specializes in Family Practice and is certified by the College of Family Physicians of Canada.
Dr. Nizar Guindi, MD, CCFP

Dr. Zeeshan Najam Khan, MBBS, MRCGP, CCFP
Dr. Khan is a Family Medicine Specialist in Ontario and Nova Scotia. He has extensive experience in general practice. He is licensed and registered with The College of Physicians and Surgeons of Ontario, in good standing, and is certified by the College of Family Physicians of Canada.

Maha Ghaly, R.Ph.
Maha is a Registered Pharmacist with the Ontario College of Pharmacists. Maha joined PharmaSense Pharmacy as an intern in 2010, and now works as a staff Pharmacist. Maha provides medication counselling and MedsChecks for chronic diseases and other conditions. She is also certified in administering injections and immunizations, and also in First Aid and CPR. Maha is fluent in English and also speaks Arabic.
Diana Mak, R.Ph.
Diana completed her pharmacy training at the University of Toronto in 2005 and then pursued an industrial pharmacy residency at Eli Lilly Canada. She joined Pharmasense Pharmacy on a part time basis since 2010. She enjoys working with patients and providing quality pharmaceutical care.
